Start requesting pathology with PathWest

Are you setting up a new medical practice or newly joining one? PathWest works with a range of health professionals, both in the community and within our hospital network to deliver health care to patients.

You can get started with requesting pathology with us in three simple steps:

  1. Have your Medicare Provider Number, your contact details and a list of practitioners who will want to receive reports
  2. Decide how you would like to receive reports. Consider if PathWest Direct (our electronic report delivery system) will work for you
  3. Complete a New Client Request Form or speak with our Client Liaison Team by calling 13 PATH (7284) or emailing us.

As well as providing 76 licensed collection points across Western Australia, PathWest also works with medical practices to conveniently locate collection services nearby or within your practice.

Ordering request forms and consumables

PathWest provides requesting doctors with A5 or A4 request form pads (50 forms per pad) with personalisation available on A5 forms. We also provide consumable products, in line with Health Insurance Commission (HIC) regulations, for our requesting doctors at no extra charge.

To order your request pads or consumables, please download and complete the forms below and return them to us. If you are not a regular requesting doctor, please contact our Client Liaison team first on (08) 6457 2142 or by email and we will gladly get you started.

Electronic request form

For practitioners consulting online or via telephone, please refer to the guide below when requesting pathology.

  1. Download an electronic request form and complete it on your computer
  2. Save the form and email a copy to your patient
  3. Instruct your patient to print out the form before attending their nearest PathWest collection centre.
